The Porphyrias - Overview

 A group of metabolic disorders caused by abnormal functioning of heme

biosynthesis enzymes

 Characterized by the excessive accumulation and excretion of porphyrins

and their precursors

 Incidence - 5 - 10 in 100,000  Long periods of latency (Up to 50% of adults). The risk to develop

symptoms depends on age, sex, exposure to provoking agents, and other factors.

The Porphyrias - Overview

 Acute porphyrias present with a variety of nonspecific neurovisceral

symptoms - abdominal pain (most common), psychiatric disorders, neurologic symptoms.

 The cutaneous porphyrias - affecting sun-exposed skin.

Clinical manifestation

 Acute attacks are reversible if treated

promptly.

 Hepatocellular carcinoma is increased in

hereditary coproporphyria

 Motor neuropathy may develop in severe

attacks and may progress to respiratory failure and death.

Exacerbating factors

 Medications  Smoking  Sex hormones (eg, progesterone)  Reduced caloric or carbohydrate intake (especially fasting)  Stress  Infections  Alcohol

Treatment

 Attacks with severe features should be treated with Hemin.  Attacks that are less severe can be treated with intravenous or oral glucose  Removal of precipitating factors  Supportive care (ICU, analgesia, antiemetics, propranolol, laxatives…).  Liver transplantation
