- 12 “System Integration Teams” working throughout the
entire county
- Teams comprised of one social worker and one peer
support specialist

- Housing navigation and tenancy supportive services such as
how to be a good tenant, problem-solving
- Assistance accessing physical and behavioral health services
- Access to social services, such as benefits
- Work with the individual for two years
