1 to 6. (1: 1st choice,… and 6: 6th choice)

will commence training in wk 1, 06 Jan

centre for change of 3rd Lang day form and submit to HOD PE/CCA for endorsement

• Pl note that private tuition or classes will not

be taken into consideration as grounds of appeal or change
