Getting Started:

  • 1. Identify an agency/experience
  • 2. Make contact to arrange Volunteer Service hours
  • 3. Complete Parent Waiver Form
  • 4. Complete Log Sheet
  • 5. Complete “Personal Reflection” Sheet
  • 6. Turn in Parent Waiver Form, Personal Reflection,

and Log Sheet to Mrs. Piper

  • 7. Place copies of the above documents into your

e-portfolio (optional)
