Getting Started:
- 1. Identify an agency/experience
- 2. Make contact to arrange Volunteer Service hours
- 3. Complete Parent Waiver Form
- 4. Complete Log Sheet
- 5. Complete “Personal Reflection” Sheet
- 6. Turn in Parent Waiver Form, Personal Reflection,
and Log Sheet to Mrs. Piper
- 7. Place copies of the above documents into your
e-portfolio (optional)
Parent Waiver Form:
- Parent consent for students
to work with agencies who have not partnered with the Palisades School District
- Parental Waiver is linked
to the VSE website under “Forms”
Log Sheet:
Back - students log hours Front - student responsibilities
- Allows students to
keep track of service hours throughout high school
- Students should
include a copy of log sheet in their portfolios
