"Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke"

  • MDHHS was awarded this grant and

has asked to partner in order to help increase access to Diabetes Prevention Programs and Diabetes Self-Management Education and Support Programs in Wayne County, Saginaw County, and Northern Michigan using the GLHC referral application

  • Grant monies will be used to

implement the referral application among partners. GLHC Implementation Consultants and IT Department will provide training and support.

  • Begins January 2019
