  4. • Proof of Residency: MUST BE DATED OR IN FORCE BETWEEN FEB 1, 2017 (previous year) & FEB 1, 2018 (current year) One Document from each of the A single document may satisfy 2 three categories categories but may only be used REQUIRED to meet a single requirement: eg: municipality billed water, sewer or waste disposal Page 18 & 19 of rulebook

FAQ’s • What if my player has a signed Tournament Player Verification from a previous season? • As long as the supporting proof of residency documentation (or school enrollment form) from that year is attached it remains valid for 2018… • There is no need to renew the proof of residency documentation for the newly instituted 3 groups • Note: Original Birth Certificate nor a copy of Birth Certificate is required • Have identified errors in past TPV Forms and need to review for 2018

FAQ’s • What if my player has a signed Tournament Player Verification from a previous season – BUT his address has changed since then & yet remains within the leagues boundaries? • A New Tournament Player Verification Form must be completed accompanied by: • three new proof of residency documents meeting the Group 1,2 & 3 requirements • OR • School Enrollment Form (assuming school is w/in boundary)

FAQ’s • What if my player lived inside my leagues boundary and has Tournament Player Verification from a previous season but has moved since then and he now resides OUTSIDE the leagues boundaries? • A II(d) waiver must be completed. Requires DA Signature. • This is REQUIRED EVEN IF THE PLAYER IS NOT PARTICIPATING IN TOURNAMENT • II(d) waiver need only be completed one time and is valid as long as the player maintains contiguous annual participation w/ the league

FAQ’s • What if my player has a signed Tournament Player Verification from a previous season, previously used the school enrollment form to prove league eligibility and has changed school enrollment to a different school building (either w/in or outside of the boundary)? • A II(d) waiver must be completed. Requires DA Signature.

FAQ’s • What if my player lived OUTSIDE our boundary, previously used the school enrollment to prove league eligibility and has changed school buildings to one that is OUTSIDE the boundary? • A II(d) waiver must be completed • This is REQUIRED EVEN IF THE PLAYER IS NOT PARTICIPATING IN TOURNAMENT • II(d) waiver need only be completed one time and is valid as long as the player maintains contiguous annual participation w/ the league, and attends that school. • Should they again change school buildings another II(d) waiver would be req’d

AFFIDAVIT REVIEW GUIDELINES ➢ FOR EACH PLAYER • IF Tournament Player Verification (TPV) Form is NOT signed by Pete Kline/Bill Meyers/Josh Fritz/Joe Wendler –  Put “TPV” in margin to right of players name on affidavit  League will then need to go to the TPV – review table to have TPV form reviewed, Affidavit initialed and signed.

Group * Verity parent/guardian name matches player name * Verify Address on each POR against Address on Affidavit * Verify Date on each POR between Feb 1 2016 & Feb 1 2017 * Verify any applicable Waivers [ II(d)/IV(h)/CC] -OR-  1b. School Enrollment FORM -AND-  2. Original Birth Certificate * Verify League Age against DOB from LL Age chart:

TPV Form Review:  If 100% Complete & Accurate –  DISTRICT ADMINSTRATOR or Asst DA to sign  Initial for player on Affidavit For  If Incomplete:  DO NOT sign  On Affidavit use the following CODES – in the margin to the right of the player  RG1 – if POR is needed for GROUP 1  RG1,2 &or3 – if POR is needed from multiple GROUPS  BC – Birth Certificate is Required

Considerations to simplify  Require/Request ALL Tournament Eligible AGE players to have the school enrollment form completed during the season…well in advance of tournament time….  This only need to be done once for that players career unless they change school buildings…..then only a II(d) waiver is required  Include on League Registration form a place for school building attended…this would allow league to identify if a player changed schools
