Home from Hospital Service
Mr G is a 76 year old bachelor who lives alone in a 3 bedroomed house; he sees his elderly sister occasionally. He admits to being socially isolated and having feelings
- f loneliness. Until recently he drove his car locally and was able to do most daily
- tasks. He suffers from Parkinson’s disease and osteoporosis which has led to a
quite marked curvature of his spine. In November 2018 he had a fall at home and following a short hospital stay he was discharged home with a Zimmer frame and some community therapy. A referral was made to the HfH team and an assessment was carried out. It was apparent that he had some considerable unmet needs. How we supported Mr G:
- Attendance Allowance Application
- Befriending Visits
- Community Visits to Dominoes and Scrabble
- Wiltshire Farm Foods
- Care Line
- Ring & Ride
Needless to say Mr G has a safer and brighter future to look forward to.
