Context Analysis
- The Burning Platform: 10 Months to 1st phase of major service
transfers into new Acute PFI and 90 weeks for delivery of the economy wide service change plans.
- Financial climate – commissioners and providers facing
unprecedented challenges
- Organisational Change - Need to stay focused on the delivery of
service transformation, improved quality and productivity – aligning all to Quality, Innovation, Productivity and Prevention
- A drive for care outside of hospital where this is safe, cost effective
and affordable
- Aligning the programme with the White Paper “Liberating the NHS”
ie the new role of GPs as commissioners and Consortia and the role
- f Local Authorities in strengthening public health, improving health
and wellbeing.
What is planned?
- £400m PFI Acute Hospital - Single site, modern, 293 less beds
9 Community Facilities delivering range of services including diagnostics, assessment, outpatient clinics.
- Re-specification of Community Beds to ensure effective step down of
patients from the Acute Hospital, high quality provision across agreed pathways and better value services.
- A strengthened Community Single Point of Care to support effective
access to the range of community services including rapid response (2hrs) and access to intermediate care provision.
- Strengthened Care coordination for patients with Long Term Conditions
with access to specialist support when needed in specific disease groups eg Chronic Obstructive Pulmonary Disorder

Progress at January 2011
- Bed Reduction Plan in on track for March 2011 = 99 beds
- Acute Hospital making good progress reducing length of stay against
best practice with no evidence of increased readmission
eg #Neck of Femur from 11.5 to 6 days, Hip Replacement 10.7 to 5 days
- Admission Avoidance – 2 major service changes were launched in
October 2010 and include the Single Point of Care and Emergency Dept / Ambulatory Care
- Community Beds/Places – All bed capacity is fully operational and
providers are making progress in improving their efficient use of this bed based service and the better flow between the acute and community.
- Outpatients – Finalising plans for redesign and care setting. Avoiding
unnecessary appointments through use of clinical protocols etc.
