What to bring to Enrollment Night
RESIDENCE VERIFICATION IMMUNIZATION RECORDS Enrollment Confirmation Course selection sheet
- Students with IEP's will have a transition IEP meeting held at
your middle school prior to the end of this school year. Accommodations and services will be discussed at this meeting.
- Currently, RHS staff is collecting recommendations and
feedback from staff at the middle schools regarding academic class placements for students with an IEP.
- Our Program Specialist will be present at the enrollment
nights to answer questions.
- Please let the staff you are meeting with during the
enrollment night know if your student is on an IEP.

- If your student has a medical plan or medication needs,
please see the school nurse before the start of the school year.
- Students with diabetes, food allergies, or seizure disorders
will need an emergency plan.
- Medications of any kind will need prior MD and parent
approval for students.

As of July 1, 2019, ALL students in K-12 will now be required to have, at the time of enrollment:
- Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
- Polio (OPV or IPV) — 4 doses
- Hepatitis B — 3 doses
- Measles, Mumps, and Rubella (MMR) — 2 doses
- Varicella (Chickenpox) — 2 doses
ACCEPTABLE Immunization Documents: Document must contain name and DOB
- Yellow Immunization card
- MD/Healthcare Provider printout
- Copy of Blue CA Immunization Card (must be signed)
- CAIR printout
Printouts from previous schools, unfortunately, do not meet California state requirements.
