Return the Consent Form with a copy of your After School Programme Contd.. child’s Baptism Certificate and payment to:  Religious Instruction will be provided by a catechist for 1 hour per week. ( Double Classes ) Parish Office FAO: Sacramental Preparation Programme  Class Time: Catechist’s availability. (ETNS/GSM), Frederick Street,  Parents contribute to the cost of the instruction. Ashbourne,  € 110 per annum paid in two instalments: Co. Meath. - First payment ( € 55) with completed Registration Closing Date: Form. Friday 3 rd May 2019. - Second payment ( € 55) in February 2020. Code of Behaviour.  Crucifix/religious image.  Simple Prayer book.  Establish and foster links between catholic families in the school and the parish. REGISTRATION FORM FIVE Senior Infants & Fourth Class New to Programme FINGERED PRAYER CONSENT FORM Senior Infants & Fourth Class First Class & Fifth Class Sign of the Cross New to Programme Our Father Copy of child’s Baptism Certificate is required for any child not baptised in the parish.
