Requesting Non-catalog Materials

For materials/equipment/furnishings not typically found in early childhood catalogs (such as landscaping mulch, fencing, etc.):

  • 1. Talk to your QI Specialist before doing anything
  • 2. Take “before” photo(s) of area(s) in need of improvement
  • 3. Obtain 3 bids and submit to the QI Specialist
  • Bids must have the same specifications, such as same square footage, same

materials/comparable materials, etc.

  • Include W9 and contact information for each vendor
  • 4. Submit grant form to QI Specialist for approval and submission
  • 5. QUALITYstarsNY will notify you if and when the work can begin

Processing Requests How it works

  • QI Specialist approves/denies request
  • Provides feedback, if denied
  • QI Specialist submits request and shopping cart to QUALITYstarsNY
  • Allow 6-8 weeks for delivery

Once you have received materials at your program:

  • Inspect upon delivery to ensure everything has been included and is in good

condition

  • Scan and submit packing slips to support@qualitystarsny.org.
  • In the subject line, write “Packing slip” and include your program’s

name and QUALITYstarsNY ID!

  • Take “after” photo and send along with “before” photo to

support@qualitystarsny.org (optional but strong encouraged!)

  • Observe and determine whether materials are making the desired difference
