Population Health ▪ A higher birth rate & a higher teen birth rate ▪ A lower life expectancy (76 yrs) ▪ A higher rate of alcohol & tobacco-related hospitalizations ▪ A higher rate-adjusted mortality rate, with nearly twice the rate of deaths due to alcohol, illicit drug use, suicide, motor vehicle accidents, diabetes and lung cancer ▪ A similar prevalence of leading chronic conditions including depression/anxiety (25%), hypertension (25%) and asthma (13%) Vancouver Island North Local Area Health Profile (85) 2012 viha.ca

Better support for pts with chronic diseases & pts requiring mental health & substance use services 4. Enhance continuity & coordinated care 5. Pt-centered care, closer to home 6.
