Population Health ▪ A higher birth rate & a higher teen birth rate ▪ A lower life expectancy (76 yrs) ▪ A higher rate of alcohol & tobacco-related hospitalizations ▪ A higher rate-adjusted mortality rate, with nearly twice the rate of deaths due to alcohol, illicit drug use, suicide, motor vehicle accidents, diabetes and lung cancer ▪ A similar prevalence of leading chronic conditions including depression/anxiety (25%), hypertension (25%) and asthma (13%) Vancouver Island North Local Area Health Profile (85) 2012 viha.ca
Goals and Objectives 1. Enhance longitudinal care 2. Increase access to primary care services in remote communities 3. Better support for pts with chronic diseases & pts requiring mental health & substance use services 4. Enhance continuity & coordinated care 5. Pt-centered care, closer to home 6.
