  • The demand forecast forms the basis of Zone capacity

planning, however the math is only one part of the process – Demand forecasts at the sub-zone service area level need to be validated with front-line knowledge and experience

  • Capacity changes throughout the year feed into the next

round of CCCNA planning

Continuing Care Placement

Provincially:

  • Alberta released the “Access to a Designated Living Option in Continuing

Care” policy in 2015 – Client choice and person centredness are key priorities in the policy – Transparent and consistent process – Clients preferred choice can be anywhere in Alberta – Approval dates in one zone apply to other zones – Uses the RAI HC as the standard assessment tool in all zones – Increased movement of clients to their preferred choice

Continuing Care Placement - continued In the North Zone:

  • North Zone created a centralized placement team in 2014

– Previously there were 3 separate teams, 3 separate policies, 3 separate processes – Increased access to services for clients – Strong advocacy for the zone on provincial committees – Consistent processes across the zone in delivering services – Creation of partnerships with other Zones – Emergency response improvement – Ability to learn from each other, and create program improvements – Ability to ensure clients are treated equally regardless of geography

North Zone Continuing Care Access 1-855-371-4122

  • North Zone staff provide information, navigation, access

and intake into continuing care

  • 7 day per week staffed with RN’s
  • Inquiries for information or entry to the health system for

home care, supportive living or long term care

  • Consistent assessment tool used when working with

getting the client access to the services they need

Palliative Care in the North

  • Program planning to develop a palliative care program

to support clients across the zone

  • Key goals:

– Allow clients the opportunities to make choices – Support service empowering front line care – Front line education will be a key feature
