You May Continue Your Coverage

- When coverage is effective and would otherwise terminate because
you end employment with your employer, coverage may be continued.
- You may continue the coverage that is in force on the date your
employment ends, including dependent coverage that is in effect.
- Coverage may not be continued if you fail to pay any required
premium.
- Premium for continued coverage is paid directly by you.

- Payable once per calendar year
- Payable for wellness tests performed as the result of preventive care,
including tests and diagnostic procedures ordered in connection with routine examinations. Examples include:
- Annual physical exams
- Mammography

- PSA Blood Tests
- Pap Smears

Group Accident – How it Works
Accident plan is selected. You injure your leg in a covered accident and go to hospital via ambulance. ER doctor diagnoses fracture and treats you. You leave the hospital on crutches.
The Accident plan pays
$2,160
Amount payable is based on High plan benefit amounts: Closed-Reduction Leg Fracture ($960), Emergency Room Treatment with XRay ($400), three Follow-up Treatments ($300), Ambulance ($400) and Appliance (Crutches) ($100).
