• Assessment tool had to be translated into Spanish
• Time was an issue, not all assessments could be

completed during medical visit

• New tool was utilized and completed with 5

clients from 3 clinics total of 15 clients

• 2 Questions were changed with client input

• Clients took notice of new tool, asked more

questions and engaged more during assessment

• 5 out of the 15 clients were referred to further

services (1 food pantry) (1 to case management) (2 to Outpatient Mental Health)

• We continue to utilize new assessment tool in all 3

clinics

• We continue to ask for client input about new tool

and will continue to make changes as needed
