- Mondays. Please ensure you measure the wound on
admission and describe it. When in doubt take a photo and document in the pressure ulcer band.ET nurses will correct it if needed. Better to err on the side of caution. Best Practice: If you identify a wound on admission. Have a second nurse assess it and document verified with_____________.
