For Your Review: 2 of the 7 acquired were related ICU respiratory devices-  Bi-Pap mask. Please ensure you assess skin under all devices .( We will be partneri tnering ng with th resp p dept. pt.) Many community acquired wounds are being  documented in the skin band and not the pressure ulcer band Measurements are not recorded on admission and on  Mondays. Please ensure you measure the wound on admission and describe it. When in doubt take a photo and document in the pressure ulcer band.ET nurses will correct it if needed. Better to err on the side of caution. Best Practice: If you identify a wound on admission.  Have a second nurse assess it and document verified with_____________.
