Nicole Banister—Deputy Director

Nicole Banister, MHA, FACHE, is the deputy project director for the CPC+ Regional Learning Network at TMF Health Quality

  • Institute. She manages practice transformation efforts across all

14 CPC+ regions. As part of her work, Nicole facilitates the learning and diffusion of best practices across 3,000 primary care sites and for 50+ payers, all of whom are working together to achieve care delivery transformation that supports the CPC+ aims of achieving better care, smarter spending, and healthier

  • people. Prior to her work with TMF Health Quality Institute,

Nicole held leadership positions in hospital systems and physician practices that were focused on health care operations, quality improvement, and value-based care. Fun fact: Nicole is an avid trail runner and enjoys camping with her family.

Sandi De Angelo— Learning System Manager

Sandi DeAngelo is the learning system manager for the CPC+ Regional Learning Network at The Lewin Group. For each of the 14 CPC+ regions, Sandi oversees the development and delivery

  • f learning events, including coaching activities, regional learning

communities, and learning sessions. In addition, she works with Wendy Bradley (TMF) to coordinate activities with the National Learning Community. Prior to the CPC+ initiative, Sandi worked

  • n several learning systems for the Center for Medicare &

Medicaid Innovation, including the following models: Comprehensive Joint Replacement, Strong Start for Mothers and Newborns, Health Care Awards, and the Medicare Care Choice

Fun Fact: Sandi graduated with a high school class of 26, so she has an appreciation for the uniqueness of rural communities, specifically in regards to health care and education.

Wendy Bradley—Knowledge Management and Spread Coordinator

Wendy D. Bradley, LPC, CCSAC, is the knowledge management and spread coordinator for the CPC+ Regional Learning Network. She is currently the director of behavioral health integration at TMF Health Quality Institute. Wendy has

  • ver thirteen years of experience in primary care

transformation, much of which has been in the implementation and oversight of behavioral health integration, team-based care, management of high-risk patients, and community engagement approaches to addressing determinants of health. She also serves as a faculty member for the Institute of Health Care Improvement and has supported practices across the U.S. and Europe as they shift toward a whole-person/whole-team approach to health care. Fun Fact: Wendy loves to travel and has lived in seven U.S. states and abroad in one other country.

Holly Phipps—Assistant Learning System Manager

Holly Phipps, MSW, former Eastern Region Cluster Lead is currently working to help support RCLs and CLCs across the marketplaces in CPC+. She is a consultant with The Lewin Group and has worked on various CMMI Learning Systems during her two years with the firm, developing a level of expertise through her work managing and planning learning events to developing qualitative data reports. Prior to her work at Lewin, Holly taught elementary school for several years and worked in global mental health to develop training curriculum for medical providers in Haiti as they built a behavioral healthcare system. Fun Fact: Holly grew up on a farm and has a potbelly pig named Charlotte.

Meagan Alley—Cluster Lead

Meagan Alley, MPH, Western Region Cluster Lead is currently working to help support RCLs and CLCs across the marketplaces in CPC+. She is a consultant with The Lewin Group and has worked on various CMMI projects during her four years with the firm, such as the Health Care Innovations Awards (HICA), developing a level of expertise through her work managing rapid cycle report

  • production. She also investigated nontraditional patterns

plan requirements. Fun Fact: Meagan hails from Louisiana and Texas and is still not used to DC-area winters.

Mira Friedlander—Communications and Logistics Coordinator

Mira Friedlander, BA is the Communications and Logistics Coordinator for the Western cluster. She recently came to Lewin from Medica, where she specialized in Minnesota State Medicaid health

  • plans. Prior to that, she did a year of service for

Americorps, serving low income families and promoting nutritional education, cooking skills, and a healthy lifestyle through University of Minnesota Extension and Share Our Strength. Fun fact: Mira enjoys rollerblading around the many lakes in Minnesota with her puppy Ryder.

Kristen Schuster—Practice Facilitator

Kristen has spent the last 5 years working in primary care as a practice transformation specialist. She has focused on quality improvement initiatives including Patient-Centered Medical Home(PCMH), Physician Quality Reporting System(PQRS), and Meaningful Use. In addition, she is a super utilizer for eClinicalWorks and specializes in building infrastructure within an electronic medical record in order to capture and report clinical quality data.

Patty Kosednar—Practice Facilitator

Patty is a certified Project Management Professional (PMP) certified EHR professional and a HIMSS certified professional in Health Information Management

implementation of EHRs, reporting for CMS Programs (MU, PQRS, IQR), electronic clinical quality improvement and workflow analysis and process improvement.

Sarah Leake—Practice Facilitator

Sarah joined Mountain-Pacific Quality Health in 2010 and is a Health Technology and Quality Consultant. Her professional experience includes over 25 years in application and system She has provided planning, assessment and guidance to rural providers, clinics and hospitals to successfully participate in CMS Meaningful Use and Quality programs. She has worked with Indian Health Services the last five years help in navigating through these program requirements. Sarah has a B.S. in Industrial Engineering from Montana State - Bozeman, MBA from University of San Diego and Healthcare IT certification from UC San Diego.

Amber Rogers—Practice Facilitator

Amber Rogers RN MSN has been an employee at Mountain-Pacific Quality Health since 2015. Her responsibilities include development of community coalitions to decrease gaps in care transitions, navigating the healthcare transition from volume to value and implementing strategies to improve prevention measures- specifically adult immunizations. Prior to employment at Mountain-Pacific Amber was a director at a physician hospital organization where she oversaw grant programs, physician peer review program and development of a clinical quality program. The grants that were implemented were focused towards support of the critical access hospitals in order to improve clinical education, EHR implementation and implementation of integrated behavioral health. Amber also has experience as a nursing director at a PPS with a variety of clinical departments and as the chief nursing officer at a CAH.

  • Q: Have I missed anything? Absolutely not. This is the official launch for your
  • market. We will have 2 more sessions. During this time, we will work on further

developing the regional learning support network.

  • Q: Where do I begin? Getting up to speed on the intent and goals of the

program is your first step. Critical readings include the CPC+ Phase 1 Implementation Guide (Including the Change Package), CPC + Payment Methodologies, and the CPC+ Care Delivery Reporting Requirements. In addition, you will want to ensure that someone in your practice has access to the CPC+ Practice Portal and that those involved in leading change also have access to the CPC+ Connect site. Links to both and to support are on the previous slide.

  • Q: What is expected of me this first quarter? The portal appears to ask

for a lot this first quarter. Am I expected to get all of this done at once? The intent of the practice portal is to collect data on what you are working on

  • already. The intent is not for you to begin doing everything at once. This is a

large program and implementing these changes will take time. Practice changes (workflow, staffing, etc.) should be intentional and driven by data.

  • Q: How many eCQM measures do I have to report? The requirement is to

report 9 of the 14 practice level eCQMs from your Certified Health IT Software Please see the CPC+ Quality Reporting Overview for Performance Year (PY) 2017 document for more details.

Role of Leadership in Transformation

In order to successfully drive improvement efforts forward the Practice Lead and Clinical Champion must be clearly identified and involved in leading the change.

  • Practice Lead Role

– Evaluate system/ practice needs – Determine priorities and align system around goals/projects – Take responsibility for portfolio of projects – Develop internal improvement capacity

  • Clinical Champion

– Lead change efforts through initial testing and iteration (early adapters) – Provide feedback on changes – Communication to clinical teams about changes – Lead Development of clinical process and protocols
