Laying the contextual framework for HCBS initiatives

  • Needed to secure legislative support for funding to shift HCBS services in new

directions

  • Systems had shared goal to generate community buy in around HCBS initiatives
  • HCBS Summit held in December 2016
  • Created awareness amongst stakeholders of the CMS Final Rule community integration

requirements

  • Provided an overview of the transformative changes and new frameworks occurring

within HCBS programs for person-centered thinking and supports to family

  • Highlighted the overlap of the HCBS requirements across the different service systems

Building a Foundation of Person Centeredness

  • Contracted with Support Development Associates to provide training

in Person Centered Thinking to stakeholders statewide Foundations of Person Centered Practice Person Centered Organizations

  • Co-sponsored and hosted by three (3) state

systems (DD Division, No Wrong Door, Med- QUEST)

  • Multiple sessions held over several days to

reach statewide audience across 5 islands

  • Over 300 staff representing 40+ organizations

from the various systems attended live training sessions

  • DDD will be launching Person Centered

Organizations training this fall

  • DDD will be participating with DD service

providers in this endeavor

  • Commitment to lead by example; walk the

walk, not just talk the talk

  • Cross systems participation is being

explored

Challenges Ahead

  • Generating buy-in from staff and stakeholders who are already feeling overwhelmed

by other HCBS initiatives

  • Incorporating the unique cultural context of families in Hawaii
  • Getting all state partners to the table; getting agreement around next steps
  • Determining how the LifeCourse framework fits into initiatives outside of the DDD

and aligns with other State priorities and projects

  • Coordinating the Hawaii CoP initiatives to coincide with multiple and competing

timeframes for implementation across the state systems

  • Ensuring alignment of all initiatives involving Hawaii CoP teams

 Systems are in agreement that service system silos need to be replaced with

integrated supports; however, the specifics of the process are still being defined

 Redefining leadership roles have changed based on which initiatives are involved  Ongoing discussions continue to address how to achieve new levels of integration

and how the LifeCourse Framework can be used across various state initiatives
