  8. } Assist CFI consumers and referred Fulton county residents in choosing, getting and keeping jobs. } Offer transitional, supported, and competitive employment as well volunteer positions. } Provides instruction on job- related activities, such as job readiness and completing applications.

  9. } Intensive Case Management (ICM) provides coordinated care for those who are currently served in State Psychiatric Hospitals, frequently admitted to State Psychiatric Hospitals, have been in a correctional facility, who are seen frequently in Emergency Rooms; or who are chronically homeless. } Services are focused on assessing needs, obtaining services, obtaining treatment and supports, and in preventing and managing crisis. } With a focus on rehabilitation, interventions are delivered primarily in the community rather than in office setting.

  10. } Homeless Outreach and Case Management Services are provided to individuals with a mental illness who are also homeless. The focus of service is on establishing trusting relationships, assessing needs, providing referral information and coordinating linkages to resources. } Activities include coordination of housing and mental health services, assistance with financial issues, assistance addressing co-occurring disorders, family reunification, and assistance addressing physical health issues and employment issues. } This program is funded by DBHDD, through a grant provided by The Substance Abuse and Mental Health Services Administration (SAMHSA).

  11. } Group Homes (Intensive Residential) } HUD Apartments (Semi-Independent) } Independent apartments(Independent ) } Phoenix House } O’Hern House } Presley Woods } Rosalynn Apartments

  12. } At each residential site, each resident has a lease in their name. } Each resident chooses their individual life goals and work with staff to reach those goals }

  13. } Located in Fulton and DeKalb counties. } Provides a high degree of supervision and support.(R3) } Teaches skills such as self-administration of medications, meal planning and preparation, housekeeping, money management, etc.

  14. } Apartments available to adults with long-term psychiatric disabilities who have a goal of living independently. } Resident manager is available during afternoon and evening hours for support and emergencies. } Residential Services staff provide assistance in skills training, shopping, recreational activities, etc.

  15. } Apartments that are located in established apartment communities throughout the Atlanta area. } Residents in these apartments typically do not need 24 hour supervision. } Housing staff schedules weekly visits for supervision/support when needed.

  18. } Opened its doors in December of 1993 as a “Low-Demand program } This now is called “Housing First”. } Provides Supportive Housing services to persons diagnosed with severe, chronic mental illness and that have been homeless. } Capacity is 76 residents.

  20. } Staff is available 24/7 to provide skills training, interventions, and other supports } Supports and services that are provided include: self-administration of medication, links to community mental health and medical services, individual skill building, recovery education classes, and a wealth of other supports and informational groups } O’Hern has a full service kitchen which provides 3 meals plus a snack since the rooms do not have individual kitchens.
