  2. What is EIDM? • CMS has established the Enterprise Identity Management (EIDM) website to provide business partners with a means to apply for, obtain approval and receive a single user ID they can use to access one or more CMS applications 2

  4. Migrating from IACS to EIDM for existing users • If you already have an EIDM account or do not have an IACS account, this how-to webinar is not applicable to you • Existing IACS accounts were migrated to EIDM on July 13, 2015 4

User Roles • The role you had in IACS will be the same role you have in EIDM – Security Official • Register to participate as a GPRO • Obtain the MYQRUR, Annual QRUR and Feedback Reports • Submit an Informal Review Request • Approve requests for Group Representative role – Group Representative • Register to participate as a GPRO • Obtain the MYQRUR, Annual QRUR, and Feedback Reports • Submit an Informal Review Request – Individual Practitioner • Obtain the MYQRUR, Annual QRUR, and Feedback Reports • Approve requests for Individual Practitioner Representative role – Individual Practitioner Representative • Obtain the MYQRUR, Annual QRUR, and Feedback Reports 5

  6. Step-by-Step Instructions • You have twenty-five (25) minutes to complete each screen (unless a different time is noted on the screen) • Otherwise, you will lose all of the information you filled in and will need to start the process again 6

  7. Step 1 • Go to https://portal.cms.gov and select Login to CMS Secure Portal 7

  8. Step 2 • Read the Terms and Conditions and select I Accept to continue 8

  9. Step 3 • Enter the following Information and select Log In :  IACS User ID  IACS Password 9

  10. Step 4 • Select Next on the Welcome to EIDM screen to begin creating your EIDM account 10

  11. Step 5 • Read the Terms and Conditions. Select the I agree to the terms and conditions checkbox and select Next Note : Next will be enabled only after checking the I agree to the terms and conditions checkbox 11

  12. Step 6 • In the Your Information section, the following fields will be pre- populated with the corresponding information from your IACS account:  First Name  Last Name  Email Address  Confirm Email Address  Social Security Number  Date of Birth • Review all pre-populated information and enter the following required information:  Home Address Line 1  City  State  Zip Code  Primary Phone Number • After all required fields are completed, select Next 12

  14. Step 7 • Create your EIDM Password • Select and provide the answer to 3 challenge questions and Next 14

  15. Step 8 • Your registration for an EIDM account is now complete. You will receive an email acknowledging your successful account creation with your EIDM User ID • Select OK to navigate to the CMS Enterprise Portal in order to access the Physician Quality and Value Programs application 15

  16. Multi-Factor Authentication (MFA) • MFA is an approach to security authentication which requires users to provide more than one form of verification in order to prove their identity. MFA registration is required only once when you are requesting a user role, but will be verified every time you log into the CMS Enterprise Portal 16

  17. Step 9 • Go back to https://portal.cms.gov and log in to the CMS Enterprise Portal • Enter EIDM User ID and Password 17

  18. Step 10 Select Username and then select My Profile from drop-down menu 18

  20. Step 12 • Read the Register Your Phone, Computer or email notification and then select an option from the Credential Type drop-down menu 20

  23. Step 14 • Your registration for the Multi-Factor Authentication is now complete. Select OK to proceed to the My Profile landing screen. 23
