- The Grantee Report: Information regarding the recipient of the funds.
This can be New York State (Part B); New York City (Part A); or an individual provider that is directly funded (Part C or Part D)
- The Provider Report: Information on each provider receiving a
subcontract from the major grantees such as NYS (all of our Part B providers and contractors)
- The Eligible Scope Client Level Data (CLD): Information from each provider
- n the clients who received the types of services that RW Funds during the
reporting period. This is a new scope of clients from previous reporting periods.

services entered into the AIRS database.
- It includes demographics, service data and clinical status measures.
- Medical Clients require the greatest level of detail and follow up.

- <435% Federal Poverty Level (FPL)
- New York State Resident

Data Verification When data has not changed from one reporting period to the next reporting period, agencies may have the option of marking select records as “Verified”.

Key Points of Data Verification!
- Only allowed for two years from Effective Date/Date Asked.
- There is a limited number of verifications allowed for each
record.
- Verified Records CANNOT be deleted/removed!

- If you are not sure if a record should be verified…
