MEDICALLY FRAGILE AND AGED DAY SUPPORT PROGRAM

Center-based Day Support and Community Engagement for individuals with significant healthcare needs

• Highly individualized and based on each

• Full time Registered Nurse, part time Registered

nurse, Licensed Practical Nurse and CNA/Aides

• Provision of individualized medical care
• Sensory stimulation
• Development of task learning skills
• Meaningful community integration and reverse

GROUP SUPPORTED EMPLOYMENT

Opportunities to try a variety of work Daily training and support at the worksite by qualified staff Paid vacation/sick days and holidays Transportation services from home to work for most participants Pay based on productivity using approved wage rates Help in achieving individual goals Opportunities to access the community Advocacy support

INDIVIDUAL SUPPORTED EMPLOYMENT
