The purpose of this grant program is to assist Critical Access Hospitals (CAHs) by providing funding to state governments to encourage quality and performance improvement activities. It is through HomeTown Health’s valued relationship with the Florida State Office of Rural Health that we have been able to provide support and education to the FLEX Collaborative from 2012-2015.
