Group of Norfolk) as a major partner
- Range of partners included in the bid writing process – including 3
different day care providers
- Existing NCC Transition team data used to guide initial target areas
- Young People currently going through or just gone through transition into
adult services and their families were also consulted
- Sport England Core Market Fund – 4 year project, started July 2018
- Targeted around Transition; young disabled adults leaving education and moving
into adult life and how sporting activity level drops during this process
- Supporting Day Care Providers to deliver more Physical Activity on site – both via
delivery & training. Target of 56 across 4 years

‘On The Move’ with Assist Trust
Since a previous sports group engaged with the ‘On The Move’ program, we’ve noticed;
- Increased confidence to try new
things/different activities and groups at Assist (not sports related).
- Increased confidence/ability to follow
instructions, which has aided certain members with developing their employment skills.
- Noticeably improved team work skills and
positivity in encouraging others.
- More willingness and desire to participate
in sports.

Came to HCH on a weekly basis for 10 weeks, providing enjoyable exercises. Those sessions were fun and the games provided physical interactions as a group. The sessions showed that they could work “as a team” and still have fun
Every Customer was able to join in the activities that were provided
The equipment enabled the sessions to be fun. Exercising became enjoyable for those who avoided it in the past.

Those that felt that they were unable to take part in the activities soon realised that they were able to accomplish and experience fun through exercise.
Active Norfolk were very friendly, helpful and supportive.
