  2. Demonstration Site Objectives • Supplement ongoing climate change and health adaptation initiatives in local health departments. • Provide technical assistance to facilitate successful implementation of climate and health initiatives.

  3. Eligibility • Local government entity or other local organization working in climate change and health. “Local” is here defined as organizations working at county, city, or community levels (e.g., local health departments). • Have experience working in local climate change and health initiatives. • Have current projects in climate and health that would benefit from supplemental funding. • Demonstrate need for funding. Priority will be given to applicants who: Have experience working with NACCHO on climate-related projects.

  4. Application Procedure • Application • Complete the fillable PDF • Deadline: 11:59 PM ET, Thursday, October 22, 2020 to Amy Chang at achang@naccho.org. • Awardees will be notified by November 2, 2020. • All applicants will be notified by November 9, 2020.

  5. Applications Application forms: • Address every section of the application. • Be concise, but thorough. • Application will only be accepted via email and in the PDF application. • Letters of support are required for applicants that are not an employee of a local health department.

  6. NACCHO Contract Review NACCHO’s standard contract language – make sure your agency would be able to sign and return to NACCHO within 30 days of receiving contract. (Note: NACCHO has a specific contract template as approved by the State’s General Counsel for applicants from the states of FL and TX. Please email us for a copy should you need it.)

  7. Risk Assessment Form • Ability to complete NACCHO’s Organizational Subrecipient Risk Assessment Form. • Agencies that cannot complete NACCHO’s subrecipient risk assessment form should not apply for this initiative.

  8. Budgets Proposed budgets should range from $16,000 to $24,000. Funding… • Cannot be used for the purchase or upkeep of office equipment. • Cannot be used to purchase food or beverages. • Cannot be used for the purchase of gift cards. • Previous participants have sought reimbursement for staff training, field supplies, staff time, and consultants.

  9. Timeline Project Period: November 2, 2020 to June 30, 2021 Deadlines to submit reimbursements: Invoice Period 1: 11/2/2020-1/31/2021 Due 2/15/2021 Invoice Period 2: 2/1/2021-4/30/2021 Due 5/15/2021 Invoice Period 3: 5/1/2021-6/30/2021 Due 7/15/2021

  10. Showcasing work • Participate in at least 3 bi-monthly check-in calls with NACCHO and CDC • Final report detailing your process and deliverables. • Share your work and accomplishments through NACCHO’s Stories from the Field (https://www.nacchostories.org/tell-your-story/). • Present about your work and accomplishments through a NACCHO webinar.
