  2. Eligibility & Enrollment ELIGIBILITY • Must be actively at work on the plan effective date for new benefits to be effective. ENROLLMENT • New Hires have 31 days from your date of hire to make benefit elections.  Remember , after 31 days, you will not be able to make a change to your benefits unless you have a Qualifying Life Event. If you are eligible for benefits, but wish to DECLINE coverage, please complete the online enrollment either on your work or home computer. Under each option, you will need to select “waive.” You must still complete the beneficiary information.

Sign up to receive a call from your FFGA Representative by visiting, https://desotoisd.timetap.com/ • To view available times please click “next” at the bottom right of the first 3 pages • You will then arrive at a page with a calendar showing dates with availability to schedule your call • Select the day you would like to speak with a First Financial Representative • Then select a morning or afternoon appointment window. (Please note: You are scheduling to receive a call at some point within your requested window) • Once you have made your selection, click “Next” and enter your contact information • When you are done, click “Save”. Please wait until you receive email confirmation that your appointment was scheduled

  5. TRS-ActiveCare introduces the NEW Primary Plan and the Primary+ Plan (formerly the Select Plan)! Primary+ Primary Both plans offer affordable co-pays. These do NOT offer Out-of-Network benefits, so you MUST choose a Primary Care Physician (PCP) and only go to providers in the statewide network.

  6. HD TRS-ActiveCare Plan Options HD is a PPO plan with a Nationwide network. You do NOT need a PCP on these plans. They do both offer Out-of-Network benefits, but you will save money by staying in network. HD is the only TRS- ActiveCare plan that is HSA eligible, as it does meet the IRS definition of a High Deductible Health Plan.

  15. - BCBSTX EMPLOYER PAID TERM LIFE INSURANCE DeSoto ISD provides all eligible employees a $10,000 basic life plan. The cost of this policy is paid for 100% by your employer . This is a term life policy that is in effect while you are employed. VOLUNTARY TERM LIFE INSURANCE In addition to the policy provided by the district, you can purchase additional term life insurance through Blue Cross Blue Shield of Texas. Voluntary Term life premiums are “age-banded” and will increase as you age into the next 5-year age-band tier. This plan is portable, but rates may be higher than your current rates.
