Instructions: Click on your math course after logging in Sample Screen Shot (Make sure course name is correct)
Answer all questions honestly, please fill in free response boxes as well. All questions should be answered, including #13 and #14.
Go back to Home Screen • Click on Completed Survey • Go to my course and click on the words “send proof” . • I will receive an e-mail stating that you have completed the course survey. • The e-mail will only state the date and time of completion. • The results will not be available to instructors until after grades are submitted.
