Colchester Middle School Melissa Goldberg RN Focusing on Immunization ● 98.7% compliance with the VT Department of 2015-2016 Enrollment: 425 Health 2016-2017 Enrollment: 444 Wellness at Work ● Served as a PATH leader and coordinated a School Year Office Visits “Biggest Loser” competition which was a wellness promotion. 2015-2016 4,389 Created several care plan templates in Google ● 2016-2017 4,111 docs for sharing across the district.

Colchester High School Deborah Deschamps RN Patty Morel LPN Focusing on Immunization 2015-2016 Enrollment: 734 ● Achieved a compliance rate of 98.8% through education and persistence. 2016-2017 Enrollment: 658 Collaboration with State Agencies ● CSD secured a $17,000 grant in an effort to provide an electronic health record which will collect data, School Year Office Visits complete assessments and documentation, 2015-2016 3,383 measure outcomes, note trends, and prepare communications with families. We will begin using 2016-2017 3,058 our new SNAP program this fall. One focus will be to promote yearly well-child exams.
