Two 3 bed fmats for four PMLD clients, which come with track hoists and wet rooms and assistive technology. Each fmat will have their own live in carer and be able to access two fmoating support workers to help with double handed care. These service users would attend Coburg Day Centre and stafg from the centre could provide additional support at peak times, as they have the skills and will know the clients well.

Two 2 bed fmats for four transitions clients with moderate support needs. They will live more independently and access shared community support for 8 hours per day and utilise assistive technology. It is this cohort who provide the signifjcant savings as it would be a sizeable reduction in this cohort’s current care packages. Their lower support needs reduce the risks associated with stacking a scheme with all complex clients.
