Hay suficiente evidencia sobre lo que puede funcionar … What we need are strategies to adapt and use successful interventions in targeted settings (e.g., schools, workplaces, health care facilities, public health departments) to sustain improvements to population health. CHRODIS+ is developing and testing an Implementation Strategy aimed at helping organizations and agencies involved in NCD to implement locally good practices which have demonstrated success in other settings and jurisdictions. Integrated Care Model for Multimorbidity Projects implemented by 5 organisations test the Multimorbidity Integrated Care Model Model developed by JA-CHRODIS 16 components of the model are classified under 5domains: Self Information Social and Delivery of Decision Management Systems and Community Care Support Support Technology Resources The implementation projects aim to prove the applicability of the model across different European healthcare settings.
