  5. Follow-up and Support  50+ specialist clinics under the auspices of the NHNN – consultant/nurse/physio/psych/dietician led  Dedicated clinic for young people transitioning to adult neuromuscular service  Telephone advice and support  Patient information days 5

Young Person’s Neuromuscular Clinic  2 x Neurologists, Specialist Nurse, Neurophysiotherapist  Outpatient clinics 2 x month and telephone support  Therapy referrals:  Physio/OT  SALT  Orthotics Clinic  Vocational Rehabilitation  Additional support via NMCCC – routine investigations  More involved in decisions relating to your care  Confidentiality 6
