New Hires: You are eligible 1st of the month following 60 days of employment. If you miss that date, you have to wait until Open Enrollment- September 1st, unless you experience a qualifying event. If you aren’t sure, please contact HR right away! Please refer to the BLUE AND WHITE PACKET sent to you 30-60 days after hire. If you need any help completing forms, call HR or see your office manager on site. Existing Employees: If you need to make changes or enroll, please contact HR. In most cases you will have to wait until 9/01, however if you have experienced a qualifying event, we may be able to add you or make changes sooner. Please remember – although Guardian has IN and OUT of network coverage, there is a tremendous amount of money to be saved by using participating providers. Contact HR for assistance or visit the carrier’s website for a provider directory. These plans are brokered by Cool Insuring.
Group Life Insurance: Is provided to all full-time employees at no cost to
- you. Now is a great time to update your beneficiaries, or to make sure
you have a beneficiary form on file. If you are a New Hire, there is a place to add beneficiaries on your Guardian enrollment form. There is also a separate form in your folder for your convienience. Contact HR for assistance. Voluntary Life Insurance is available from Guardian. It is virtually Guarantee Issue if you sign up on your eligibility date. If you wait, you can still enroll, however there will be additional forms required, including the Evidence of Insurability.
Prescriptions: Prescription coverage included in your Blue Shield plan is GENERIC ONLY for a $10 copay. If your doctor prescribes you a non-generic, and if no generic is available, please contact HR or Cool Insuring for
- assistance. In many cases there are coupons or programs available
to help reduce the cost of name brand drugs. Please do not go without! Call Cool or HR for assistance.
ALLY HEALTH Ally is our Telemedicine benefit. This is a great way for your to save
- n copays, especially evenings and weekends when your doctor’s
- ffice may be closed. If you participate in one of our Blue Shield plans,
you are enrolled. You must, however, go online or call to activate your
- account. Once activated, you can schedule an appointment (usually
within a matter of minutes) to be “seen” by a board certified
- physician. This could help you avoid a costly visit to the ER, or
