  2. What works? Early recognition • – It is important that we educate our communities • As many as 87% of sepsis cases originate in the patient’s community – EMS ‐ First responder training and sepsis alert protocols – In ‐ hospital ‐ Frequent assessment using established criteria The Surviving Sepsis Campaign Bundle: 2018 Update • – The 3 ‐ h and 6 ‐ h bundles have been combined into a single “hour ‐ 1 bundle” with the explicit intention of beginning resuscitation and management immediately. • Measure lactate level. – Remeasure if initial level is >2mmol/L • Obtain blood cultures prior to administration of antibiotics • Administer broad ‐ spectrum antibiotics • Begin rapid administration of 30ml/kg crystalloid for hypotension or lactate >4mmol/L • Apply vasopressors if patient in hypotensive during or after fluid resuscitation 2
