Forms must be submitted to obtain ID# to create a

MEDICAL FORMS
Pertaining only to health forms/Medical packet received from Admissions Office. If student is playing a sport, additional forms are required and obtained from the coach.
Student Health Screening Information Packet must be completed by parent/guardian and physician. This medical packet must be returned as soon as possible. Otherwise, the date of the student’s appointment must be noted on the 1st page and returned to the Admissions Office.

*Students need to fill 8 periods per day. *Course level changes from CP to Honors requires a B+ final year average in related Subject. *Additional Online electives (Distance Learning) are available to students with a 3.5 cumulative GPA in all courses. Seats are limited to 20 on a first-come, first served basis. All online course grades will count toward GPA And will be recorded on student transcripts. Interested students should meet with their guidance counselor to discuss.
