TRYCLUB (Men’s weight management)

Links to Health and Wellbeing Strategy 1. Stronger focus on prevention 2. Supporting self-management of health 3. Getting people more active, more often 4. People will live longer and healthier lives Key Priority: To engage with men to take part in regular physical activity and work towards a healthy weight. Description: Try Club is a 12-week health and wellbeing programme for men with a BMI of 25+. Split between nutrition workshops, health education and fitness sessions the group is build on camaraderie, a love of sport and empowering individuals to make long lasting lifestyle changes.

Reduce repeat Emergency visits to hospital Key Priority: Leeds will be ‘An Age friendly city where people age well’ Description: The Rhinos Active Social group meets weekly at 3 sites across the city, combining appropriate peer led physical activity with a friendship and social element. The aim of the group is to contribute towards the city's agenda for reducing social isolation, loneliness and risk of falls. *Regular activities include, tai chi, tap dancing, ping pong and walking rugby. activities include Tai Chi, Yoga, Pilates and Gentle Exercise.

IN-TOUCH (Dementia support group)

Quality of life improved due to access to services and support Key Priority: Leeds will be ‘An Age friendly city where people age well’ Description: The In-Touch group sees anyone accessing the LCC support services living with a dementia diagnosis (as well as their carers) accessing a monthly support group to socialise, relive fond memories and share stories of rugby league in the heart of their local club. Each month consists of a topic/guest speaker.

ACTIVE BEYOND CANCER (Cancer Rehab)

Key Priority: To improve the health, wellbeing and reduce the fatigue levels of living with cancer in Leeds. Description: The 12 week cancer rehabilitation programme works with clients at all stages of their cancer treatment, it looks to build strength, fitness and wellbeing through weekly sessions with the view to reduce chronic fatigue levels through a tailored and specific cancer fitness programme. 45min health and nutrition session/45min specific tailored physical activity
