- Personal Care Attendant (CNA)

What are enrollment criteria?
- Age 55 +
- Certified by the State as eligible for
nursing home level of care
- Lives in a zip code we serve
- Medicaid and Medicare eligible OR
able to pay a Medicaid monthly premium
What are enrollment criteria?
- Living in a safe and stable
environment
- Willing to participate in a coordinated
plan of care and receive all services from PACE and their contracted providers
