Care is highly-coordinated and managed Interdisciplinary Team (IDT)

• Board Certified Geriatric Physician and NP
• Nurse Case Manager
• Therapists (Physical, Occupational, Recreational)
• Registered Dietician
• Social Workers
• Chaplain
• Personal Care Attendant (CNA)

What are enrollment criteria?

• Age 55 +
• Certified by the State as eligible for

nursing home level of care

• Lives in a zip code we serve
• Medicaid and Medicare eligible OR

able to pay a Medicaid monthly premium

What are enrollment criteria?

• Living in a safe and stable

environment

• Willing to participate in a coordinated

plan of care and receive all services from PACE and their contracted providers

DM+, anticoag, chronic pain
