Flexible Benefit Allowance/Cash in Lieu Cash in Lieu Flexible Benefit If you do not enroll in medical insurance, Allowance (FBA) you will be given the Cash In Lieu instead of the Flexible Benefit Allowance If you enroll in medical coverage, $594.90 is given to help cover cost of the medical premium

Board Paid Contribution ProTech - $155 per month The Board shall pay for each full-time Support and ProTech employees, Support - $123 per month a contribution amount if the employee elects the Board’s health insurance Part time employees (4 plan to help cover the hours) - $61.50 per month cost.

  8. Health & Welfare – Major Medical Two different types of health insurance plans: • HMO Aetna HMO GlobalHealth • PPO All HealthChoice Plans

  9. Health Maintenance Organization (HMO) Healthcare services are coordinated by chosen primary care physician. • Advantages : – No deductible – Less out-of-pocket costs during the plan year • Disadvantages: – Must have a primary care physician in network – Limitations on choice in doctors and hospitals

  10. Preferred Provider Organization (PPO) PPO insurance plans allow members to choose a doctor whether in network or outside of the network. There are basic and high deductible plans. • Advantages: – Broad choice of doctors and hospitals • Disadvantages: – Deductible (what has to paid before insurance begins to pay varies depending on plans) – Greater out-of-pocket expenses

Dental & Vision Dental • Several plans offer no annual deductible – pay copays • Most plans offer free preventative care (2 annual cleanings) • Other services are shared costs – Basic Care – filings, extractions, oral surgery – Major Care – crowns, dentures, bridge – Orthodontic Care – Plan year maximum Vision • 1 annual exam per calendar year • Frames, lenses and/or contact lenses • Plans offer discount on laser correction

Enrollment Information If you add a dependent to your insurance policy, you must provide a copy of: Spouse: Marriage certificate • Dependent(s): Birth certificate for dependent child(ren), IRS taxes showing • dependents, or court order establishing legal adoption/ guardianship During the plan year, you may add or drop coverage for yourself or dependent based on the following qualifying events: Marriage/divorce • Birth/adoption/gain legal custody • Dependent’s change in status (employment) • Employee or spouse changes employment status • Death • New residence inside/outside of HMO service area • NOTE: All qualified status changes must be made within 30 days of the qualifying event and you must provide the applicable supporting documentation for qualified change.

Enrollment Information Declining Insurance : Must provide copy of insurance card • Complete declination form (included in benefits • packet) Insurance Effective Date: Insurance is effective the 1 st of the month following • the start date Example: If an employee starts work in August, • insurance will be effective on September 1

  14. Open Enrollment • Held annually in October • Changes go into effect January 1 • No qualifying event is necessary to make changes • Allows employees the opportunity to: • Add/drop/change health, dental, life or vision plans • Increase/decrease life insurance • Add/drop qualified dependents

  15. Flexible Spending Account (FSA) Allows employees to deduct premiums pre-tax basis and set aside pretax funds to use toward qualified medical and daycare expenses. You will need to contact American Fidelity to start your FSA by calling 405-416-8810 or 800-933-1853 • Flexible Spending Account (FSA)-optional Up to $2650 annually • July – June fiscal year • Use or lose plan • Funds are loaded up front • • Dependent Day Care – optional • Up to $5,000 annually

  16. Life Insurance Plans The OMES/ Standard Basic District-provided life insurance (no cost to you) Option to purchase life insurance for yourself, + spouse and/or dependent(s) Option to purchase additional life insurance

  17. Life Insurance: The Standard District-provided life insurance for each full time employee. • Certified employees -$35,000 • Support employees - $30,000 (Reduces to $10,000 at age 70) • ProTech, Central Office & Principals $50,000 Amount is based upon negotiated agreements with unions.

Life Insurance: The Standard (continued) The Standard - Must complete the beneficiary form for district paid term life insurance. _____________________________________________ Additional coverage may be purchased in units of $25,000 to a maximum of the lesser of 6 times your annual salary or $600,000. • The minimum amount to elect is $25,000 • Spousal coverage - up to $100,00 but cannot exceed 50% of your supplemental coverage • Child(ren) coverage – $10,000

  19. Life Insurance: OMES Coverage may be purchased for yourself, spouse and dependent(s): Basic Life: $20,000 for $4/month • Supplemental Life: maximum coverage is 2x • annual salary rounded to the nearest $20,000). Coverage is guaranteed as a new hire Dependent coverage: three tiers available •

Oklahoma Teachers’ Retirement (OTRS) – 401(a) Teachers Only District contributes 7% of your TRS eligible compensation Certified Employees, ProTech and Principals District contributes 7% of your TRS eligible compensation up to $60,000 Support Employees District contributes 90% of 7% of your compensation up to $40,000. You will contribute 10% of 7% contribution. Example: for every $1, the district contributes $.90 and employee contributes $.10 cents. Membership is required for exempt staff Membership is optional for support staff - must complete enrollment form and turn it in today

  22. Retirement Savings Annuity: 403b or 457b Allows an employee to make contributions pre-taxed from his or her income into a retirement plan. • Lowers taxable income • Plan for retirement • Can start plans at anytime In order to start your payroll deduction you much contact an approved vendor from the list included in your benefits packet and set up or transfer an account. It may be a good idea to consult with a financial advisor before selecting a vendor.

  23. Benefits Forms Deadline to turn in forms Must turn in forms within one week from date of onboarding • By law, you have 30 days to make a change to your elections • Takes two weeks to receive insurance cards • What happens if you do not turn in your forms within 30 days?
