SUMMARY OF FUNDING Type of Award: Cooperative Agreement Start Date: April 15, 2018 Formative Phase: April 15, 2018 – July 15, 2018 Project Period: 18 - 33 months (depending on intervention selected) T otal Funding: Up to $170,000.00 each to twenty-four (24) intervention sites; six sites per focus area

AIDS UNITED- WHAT WE DO • Lead the charge for responsive and effective federal, state, and local policies that benefit people living with and at risk for HIV • Invest in innovative models to meet the needs of communities affected by HIV/AIDS • Launch strategic grantmaking initiatives have directly funded more than $104 million to local communities, and have leveraged more than $117 million in additional investments for programs that include, but are not limited to, syringe access, access to care, capacity- building, HIV prevention and advocacy

RYAN WHITE HIV/ AIDS PROGRAM & HIV CARE CONTINUUM • The care continuum continuously evolves to reflect scientific and social advances in the strategy to fight the disease • By closely examining the proportion of people living with HIV engaged in each stage of the HIV care continuum, policymakers and service providers are able to pinpoint where gaps may exist in connecting people living with HIV to sustained, quality care, and to implement system improvements and service enhancements that better support individuals as they move from one stage in the continuum to the next.

ELIGIBILITY • Currently-funded Ryan White HIV/AIDS Program recipients or sub- recipients providing direct services, inclusive of sites providing core medical services and/or support services directly to clients • Not currently implementing the same or a similar intervention as the one selected under this RFP for funding • Must demonstrate a pre-existing relationship with an Outpatient Ambulatory Health Services (OAHS) provider or in-house OAHS that will be able to collect and report to the applicant viral load (VL) tests, retention, antiretroviral treatment (ART) adherence, etc.
