  1. The Peezy: QIPP standard right-first-time midstream, analysis, diagnosis and treatment. Giovanna Forte Managing Director Funnelly Enough Limited

  2. A Doctor’s dilemma. • In 2002, Suffolk GP Dr Vincent Forte had a difficult day in surgery • He asked five different women to provide a urine sample so he could test, diagnose and treat their problem • He gave them the standard Universal Container into which they were asked to deliver a mid-stream urine sample. They all complained (Have you tried peeing into one of these?)

  3. MSU: the start-stop-start method Hold universal container close to body (but not quite between the legs) START! STOP! Now position the container directly underneath body … AIM! GO! Wipe loo seat, legs, floor Wash hands. Wash bottle Give damp container to the nice lady on Reception

  4. MSU: the “right first time” system • Vincent decided there must be a solution to female MSU delivery • He invented Peezy, a comfortable funnel that fits neatly in place allowing the patient to “Just Go” • Peezy automatically and hygienically collects a pure mid- stream urine sample • Now patient and clinician can benefit from right-first-time sample analysis, diagnosis and treatment

  5. Why is Peezy so good? Peezy is the only collection system for women that guarantees: Right-first-time mid-stream urine sample Improved diagnosis and treatment Personal dignity and hygiene More diligent infection control Reduced revisits saving time and money for you and your patient

  6. Quality, Innovation Productivity and Prevention Peezy meets the demands of QIPP, from NHS Improvement and Efficiency and satisfies top priorities from the NHS Operating Framework 2009-10 • Improving cleanliness • Tackling healthcare associated infections • Enhancing the patient experience

  7. Peezy benefits. • NO contamination of sterile saline • NO contamination from rim to user • NO promotion or inhibition of growth of urinary pathogens (13 species tested) • NO effect on protein or glucose or HCG (pregnancy) testing in the lab • NO effect on near-patient urine multistix (dipstick testing) “If we’re looking at a cost of .99p for the right sample first time against a retest at £7.50, Peezy is a no-brainer.” Senior Ward Nurse, Lings Bar Hospital, Nottingham

  8. Peezy is approved by NHS Prescription Services and listed in the Drug Tariff from 1st May 2010. NHS DRUG TARIFF PART IXB INCONTINENCE APPLIANCES TUBING AND ACCESSORIES PEEZY: ORDER NO D950 (Peezy is also on the GP EMIS system)

  9. How to use Peezy.

  14. Positive comments (from testers at Unison Healthcare Conference 2010) • Much better than using the UC alone • Great gadget! • Useful, hygienic • Absolutely fabulous! • Think of the savings on antibiotics • Clean and easy to use

  15. Negative comments • Using a cup is easier (but won’t catch your MSU) • Fiddly to use (just hold it in place and go …!) • Too complicated for an easy task (simple hold-and-go) • Overflows if you’re bursting to go (recommend normal urge) • Urine dripped on my hand when I detached the bottle (count to 20 before lifting Peezy and removing the bottle) • Bottle fell off and into the loo (Peezy is screw-fit not push-fit)

  16. What did we learn? • 88% of uninstructed users were positive • 97% of instructed users were positive • FIRST TIME INSTRUCTIONS NEEDED

  17. Clinical Trial October-December 2010 A prospective study with historical controls to compare the relative bacterial sample contamination rates between an automated mid-stream urine collection system (AMSU) to a standard MSU collection method across all units at The Royal Free Hospital NHS Trust, and predictive antibiotic prescribing rates.

  18. What are we looking for? A reduction in the percentage of samples which are contaminated prior to bacteriological testing would result in fewer contaminated samples needing to be repeated, or resulting in inappropriate antibiotic prescribing at an additional cost to the NHS and causing delays in treating patients and potentially reducing length of hospital stay. (protocol extract)

  19. What’s the hypothesis? The use of a novel urine collection device to collect an MSU sample will reduce the contamination rate by 50% when compared with the standard MSU collection method. (protocol extract)

  20. Peezy benefits: the clinician. “We can use the Peezy for all types of patients. Pre- assessment MSUs, MSU for cultures, weekly urine testing prior to chemotherapy and so on. It has been easy to use with any age range or dexterity and proven to be cleaner than trying to get ladies to urinate directly into a bottle or risking cross contamination by using a bed pan.” Anthony Barrett Acting Ward Manager Urology North Staffordshire Hospital Trust

  21. Peezy benefits: the clinician. “I have just had a patient who used the Peezy. She suffers from recurrent UTIs, has a poor flow and found this very good and easy to use.” Deirdre McFadden Head of Urodynamics Letterkenny General Hospital, Ireland

  22. Peezy benefits: the clinician. “I saw a 60-year-old woman with a PMH of diabetes, stroke and with learning difficulties who had been troubled with urge incontinence for months. Peezy yielded the first ever urine sample from this patient followed by diagnosis and treatment. I have seen the patient since, obtained another Peezy sample and things are improving.” Barbara Oldham Practice Nurse Great Yarmouth and Waveney PCT

  23. Peezy benefits: the clinician. “My patients are delighted as it dignifies the whole procedure. It also means that I have confidence in the quality of the results.” Mr Adrian Lower Consultant Gynaecologist Women’s Health Partnership The Princess Grace Hospital London & Founder & Medical Director The London Fibroid Clinic

  24. Peezy benefits: the patient. “My daughter has learning difficulties and long-term history of chronic UTIs. Peezy helped me take the first ever mid-stream sample from her, easily, cleanly and with no fuss. Now, at last, she can be correctly diagnosed and treated. Thank you Peezy .” Daily Mail Reader February 2010

  25. Peezy benefits: the patient. “Holding the assembled kit by the bottle, I can take a sample from my child without causing distress or unwelcome contact. We can just chat and wait for "it" to happen!” Daily Mail Reader February 2010

  26. Peezy benefits: the patient. “Good! Good! Much better! Much better!” Elderly female with severe arthritis Royal Free Renal Unit 08 June 10

  27. Why women like Peezy. • There’s no mess. It’s so easy to use. Its just brilliant! • Using Peezy would give me confidence • The Peezy’s great! You can see what you’re doing, you can see there’s no mess. Its a great invention! • Using Peezy is a lot easier and a lot less of a fuss • It’s convenient, simple and easy to use

  28.
