Previous service user’s • First for Wellbeing website will provide signposting information - www.firstforwellbeing.co.uk • Customers will be able to access relevant public health information and advice, or self-refer to some key services such as weight management http://www.northamptonshire.gov.uk/healthyweight • Further lifestyle information is available via the PHE website www.nhs.uk/oneyou

Weight Management Public Health Northamptonshire will fund a 12 week course with a number of commissioned weight management services if your BMI is over 28 (conditions apply) If your BMI is over 40 you are advised to contact your GP for advice.   5. Supporting Independence Programme (SIP) To identify and support people living with mild frailty and so reduce the risk of adverse health events and subsequent demands on health and social care. Moderate Severe Fit Mild frailty 35% of O65s 12% of O65s 3% of O65s • Frailty is not a static state • People can improve or deteriorate • The majority of people living with frailty are mildly frail

Main Aims • To halt, reverse or reduce the deterioration of frailty in people living with mild frailty as measured by the Edmonton frailty scale. • To decrease the number of people living with mild frailty who experience major health events. • To support the identification and recognition of frailty in at risk patients across the Northamptonshire health and social care system. • To support the development of a system wide frailty pathway from fitness to severe frailty and end of life care. • To contribute to the knowledge base on frailty, its management and progression.

Referral Pathway Who refers? • Referrals from partners and GP’s – identified as mildly frail • Self referral How? • Visit our website: https://www3.northamptonshire.gov.uk/councilservices/h ealth/pages/supporting-independence.aspx click to complete a referral, you will be asked 7 simple questions to establish frailty needs.

  8. Frailty Assessment As part of the referral process 7 questions PRISMA 7 is a Public Health validated assessment tool: Questions Patient: Yes  No  1 Are you older than 85 years? Yes  No  2 Are you male? Yes  No  3 In general, do you have any health problems that require you to limit you activities? Yes  No  4 Do you need someone to help you on a regular basis? Yes  No  5 In general, do you have any health problems that require you to stay at home? Yes  No  6 If you need help, can you count on someone close to you? Yes  No  7 Do you regularly use a stick, walker or wheelchair to move about? Total checked:

  9. Customer Appointment • Where? Wellbeing advisor based in GP practices and community settings • How? Face to face Initial appointment - Edmonton assessment Followed by up to 12 weeks support with further in depth health assessment’s as required

Up to 12 weeks support is offered for: • Functional Performance • Edmonton – Initial • Smoking • Cognition/6CIT • Alcohol • General Health • Sensory • Falls Screening • Vaccinations • Functional Independence • Blood Pressure • Social Support • Continence • Medication Use • Edmonton - Final • Nutrition • Mood (GADS 7)

Increase the risk of falls  Balance problems/muscle weakness  Recently polished/wet floors  Poor vision  Dim room lighting  Long term conditions  Rugs/carpets that are secured properly Get Up & Go Programme 12 week strength & balance classes delivered by qualified OTAGO trained instructors. www.northamptonshiresport.org/getupandgo
