Initial Requirements of the National Information (Data) Strategy

  • 1. Empower us with data
  • 2. Let’s figure it out from there (or else someone

else will)

Industry Disruption

 Cab drivers – Uber  Travel Agents – Travelocity  Hotels – AirBnb  Stock traders – Ameritrade, etrade  Shopping malls – amazon, ebay, zappos  Grocery stores – Instacart  Movies – blockbuster – Netflix  Encyclopedia – Wikipedia

The Common Trends Across Industries

 Information is readily available  Individual experience about the service provided is widely

known.

 Service providers can be crowdsourced across the

community

 Services are provided at the convenience to the customer

slide-9

BPHC Priorities

 Maternal and Child Wellness  Addressing the Opioid Crisis  Ending the HIV Epidemic  Advancing Value Based Care  Performance and Accountability

 Health Centers  PCAs, HCCNs and NTAP (NCAs)

slide-10
SLIDE 10

BPHC and NACHC Data Projects

 UDS Modernization  Value Based 330 Funding  FARA  PRAPARE National Data System  QIDC  CDC Research

HCCN Activity Streams

 HCCN Grant  HCCN ROI  Funding Streams  National Data Strategy

SLIDE 12

National Data Strategy Feedback and Requirements from the HCCN Task Force

 Equalize each state’s capacity for data and information management  Build on existing trusted networks  Protect person privacy and health center privacy  Allow health centers to stay in control of their data  Create new funding streams for HCCNs  Build a foundation that supports all current and future clinical and

policy priorities, locally and nationally

 Develop a concept paper and then a 5 year roadmap

What a local data strategy could support:

Healthcare Priorities

Eradicating HIV Maternal and child wellness Addiction Recovery Loneliness and depression Human trafficking intervention Diabetes control Cancer detection

Policy Priorities

Impact analysis and ROI Support policy locally and nationally

Payer Priorities

Medicaid and Medicare cost control Performance reporting for accountability

Practice Transformation

Performance Support

Social Determinants

Addressing social barriers to healthcare

Environmental Determinants

Ensuring safe drinking water Connecting air quality to health events

Clinical Research

Support research networks (Million Hearts, etc)

NACHC’s Initial Policy Questions (2018)

 Which HCCNs can estimate the total cost of care per patient? Ideally, we

would like to be able to separate patients by payer type (if any) and income as a ratio to the poverty level.

 Can HCCNs compare the total cost of care for selected chronic conditions?

 Possibly by comparing patients with selected chronic illness vs. those without?

 Can HCCNs compare the total cost of care for patients with selected chronic

conditions but with varying levels of control? For example, is it possible to find out how much savings are created by lowering a diabetic patients’ HbA1c by a single percentage point? There is literature that provides evidence around amounts of cost savings generated as HbA1c decreases, so we would like to do similar analyses using health center data.

How a National Data Strategy Could Work

Empower people with a new voice against environmental influences by watching for community patterns Advance all communities. Create a sharing model allowing HCCNs to share

  • resources. Best practices in

advanced informatics. Create new funding streams from HRSA and CDC to sustain the work and use the network for all improvement strategies and supporting policy.
