Which HCCNs can estimate the total cost of care per patient? Ideally, we
would like to be able to separate patients by payer type (if any) and income as a ratio to the poverty level.
Can HCCNs compare the total cost of care for selected chronic conditions?
Possibly by comparing patients with selected chronic illness vs. those without?
Can HCCNs compare the total cost of care for patients with selected chronic
conditions but with varying levels of control? For example, is it possible to find out how much savings are created by lowering a diabetic patients’ HbA1c by a single percentage point? There is literature that provides evidence around amounts of cost savings generated as HbA1c decreases, so we would like to do similar analyses using health center data.
