Who’s Eligible

Testing is FREE for all employees, retirees, and spouses

• n the RSCCD medical insurance plan.

Your testing package is based on your age and personal risk factors for cardiovascular disease.

Services Available

Open to all eligible participants on the RSCCD Medical Plan

• Know Your Number Risk Assessment
• Disease Risk Questionnaire
• Full Lipid Panel: Glucose, Total Cholesterol, HDL, LDL, Triglycerides
• Blood Pressure
• Body Mass Index
• Personalized Report Evaluating Your 5-Year Risk for Chronic Disease
• Colorectal Cancer Testing Kit
• Easy to use home testing kit to detect blood in the stool, an early

warning sign of colorectal abnormalities including cancer

Services Available

Enrolled Individuals Age 45+, or 35-44 with 2 Cardiovascular Risk Factors

• Ultrasound Tests of the Heart & Arteries
• Echocardiogram: A video ultrasound of the heart which may detect

enlargement, valve abnormalities, blood clots, and more.

• Electrocardiogram (EKG): Provides a picture of the heart's electrical

activity which may predict pending heart attack and other problems.

• Hardening of the Arteries Test: Arteriosclerosis reduces flexibility in

the arteries which causes the heart to work harder.

• Carotid Artery (Stroke) Ultrasound: Ultrasound test to identify plaque

build-up in the arteries, a leading cause of stroke.

• Peripheral Arterial Disease Test: PAD is a common circulation

problem which increases your risk of cardiovascular disease.

• Abdominal Aortic Aneurysm Test: Checks for aneurysms in the
• abdomen. The danger lies in the risk of the aneurysm bursting

Where

• All testing takes place in the HealthFair mobile health center.
• The health center is complete with two private exam rooms.

Results

• Your results are completely CONFIDENTIAL. RSCCD nor

your insurance, will know the results of your exam.

• Testing results are reviewed by a board-certified physician.
• Results are mailed to your home in approximately 2 weeks.
• Participants are encouraged to share their results with their

primary care physician.
