  12. Your Dependent Care FSA Pre-tax account to set aside money from each paycheck for dependent care expenses. The annual IRS limit is $5,000. Accident Insurance • This plan pays out a cash benefit for qualifying accidents. • Benefits are paid directly to you or your beneficiary. • Funds can be used however they are needed — not only for emergency treatment, hospital stays and medical exams, but for other related expenses, such as transportation, lodging needs, etc. • Lump sum cash payouts vary based on each specific qualifying accident. • $100 cash benefit paid to you and any dependents on the plan per calendar year, if you go to a physician for one or more qualified health screenings. Critical Illness Insurance Critical Illness & Cancer insurance pays you a cash lump sum if you or any enrolled dependent is diagnosed with a qualifying critical illness or cancer. • Guaranteed Issue: $20,000 Employee, $10,000 Spouse, and $10,000 Children. • Available for spouse and children as well • 30 day waiting period • Fully Portable • $50 cash benefit paid to you and any dependents on the plan per calendar year, if you go to a physician for one or more qualified health screenings.

  18. Your Life Insurance Basic Life and AD&D At no cost to you, RQ provides a basic life and AD&D policy through Cigna. The life insurance benefit is equal to 1x your annual salary. All employees are eligible. You will need to enter your beneficiaries in the EMB portal.

  19. Your Life Insurance Voluntary Life Employee: apply in increments of $10,000 to a maximum of $500,000. Guarantee Issue amount up to $150,000. Spouse : apply in increments of $5,000 up to the lesser of 100% of the employee voluntary life or $150,000. Guarantee Issue amount up to $10,000. Child : apply in increments of $2,000 to a maximum of $10,000, for dependent children up until they can no longer be claimed as a full-time student. The benefit amount from birth to six months is $500. Amounts over the Guaranteed Issue limit will require Evidence of Insurability (EOI).

Your Life Insurance Whole Life • Designed to remain in force for the insured's entire lifetime. • Premiums are fixed, based on the age at issue, and do not increase with age. • Whole life insurance includes insurance and investment components: The insurance component pays when the insured individual dies. The investment component builds an accumulated cash value the insured individual can borrow against or withdraw. Please view the plan document on the ExplainMyBenefits portal for more information.

  21. Your Short-Term Disability • 100% Paid by RQ • Begins after 7 days of becoming sick or injured. (when employee is unable to work) • Pays 60% of average weekly earnings up to $2,000. Ends at the end of the 12 th week. •

  22. Your Long-Term Disability • 100% Paid by RQ • Begins after 13 weeks of becoming sick or injured. • Pays 60% of average monthly earnings up to $12,000. • Benefit period paid up to the Social Security Normal Retirement Age. (if you were born 1960 or later, this would be 67 years old)
