Some ¡organisations ¡are ¡beginning ¡to ¡introduce ¡uniforms ¡for ¡health ¡ visitors, ¡but ¡a ¡key ¡health ¡visiting ¡attribute ¡is ¡presentation ¡of ¡self. ¡ ¡Sarah ¡ Cowley ¡discusses ¡the ¡challenges ¡of ¡changes ¡that ¡are ¡introduced ¡by ¡people ¡ who ¡may ¡not ¡fully ¡understand ¡the ¡health ¡visiting ¡service. ¡ ¡

¡ April ¡is ¡the ¡month ¡when ¡we ¡expect ¡to ¡see ¡spring, ¡but ¡are ¡sometimes ¡disappointed. ¡ ¡Without ¡fail, ¡ however, ¡it ¡brings ¡changes ¡to ¡the ¡NHS ¡system ¡and ¡architecture. ¡ ¡Each ¡year, ¡there ¡are ¡new ¡names, ¡ new ¡responsibilities ¡and, ¡often, ¡new ¡organisations ¡to ¡embed ¡and ¡understand. ¡ ¡April ¡2013 ¡is ¡no ¡ exception, ¡with ¡some ¡major ¡upheavals ¡and ¡some ¡smaller ¡changes. ¡ ¡Health ¡visitors ¡in ¡England ¡will ¡ be ¡commissioned ¡through ¡the ¡new ¡NHS ¡Commissioning ¡Board, ¡to ¡be ¡known ¡now ¡as ¡NHS ¡England. ¡ ¡ One ¡of ¡the ¡workstreams ¡in ¡the ¡Health ¡Visitor ¡Implementation ¡Plan ¡2011-­‑2015 ¡(Department ¡of ¡ Health ¡2011) ¡is ¡about ¡‘aligning ¡delivery ¡systems’ ¡within ¡this ¡new ¡NHS ¡landscape, ¡so ¡ commissioning ¡will ¡be ¡overseen ¡by ¡the ¡Department ¡of ¡Health’s ¡Health ¡Visitor ¡Programme ¡Board, ¡ which ¡will ¡continue ¡with ¡refreshed ¡terms ¡and ¡membership. ¡ ¡In ¡2015, ¡commissioning ¡will ¡move ¡to ¡ local ¡authorities, ¡joining ¡other ¡public ¡health ¡colleagues, ¡including ¡school ¡nurses, ¡who ¡were ¡ moved ¡this ¡year. ¡ ¡Also, ¡instead ¡of ¡Strategic ¡Health ¡Authorities ¡and ¡Education ¡Consortia, ¡we ¡need ¡ now ¡to ¡familiarize ¡ourselves ¡with ¡Local ¡Area ¡Teams ¡(LATS), ¡which ¡are ¡each ¡expected ¡to ¡have ¡a ¡ named ¡lead ¡for ¡health ¡visiting, ¡as ¡are ¡the ¡Local ¡Education ¡and ¡Training ¡Boards ¡(LETBs). ¡ ¡ ¡

¡ 2 ¡ The ¡new ¡landscape ¡means ¡that ¡health ¡visitors ¡are ¡now ¡employed ¡in ¡a ¡variety ¡of ¡organisations, ¡ including ¡social ¡enterprises, ¡commercial ¡bodies ¡and ¡as ¡part ¡of ¡larger ¡mental ¡health ¡or ¡acute ¡ hospital ¡NHS ¡Trusts. ¡ ¡These ¡can ¡all ¡be ¡very ¡supportive ¡and ¡positive, ¡but ¡the ¡changes ¡may ¡also ¡ create ¡a ¡challenge ¡for ¡health ¡visitors ¡if ¡the ¡service ¡is ¡not ¡fully ¡understood ¡by ¡those ¡in ¡charge. ¡ ¡I ¡ was ¡reflecting ¡on ¡this ¡when ¡I ¡heard ¡from ¡someone ¡whose ¡employer ¡has ¡recently ¡required ¡all ¡ health ¡visitors ¡in ¡their ¡organisation ¡to ¡wear ¡a ¡uniform ¡for ¡work. ¡ ¡I ¡recalled ¡the ¡health ¡visitor ¡with ¡ whom ¡I ¡visited ¡as ¡student ¡nurse ¡in ¡the ¡1960s. ¡ ¡She ¡wore ¡a ¡business-­‑like ¡grey ¡suit, ¡which ¡made ¡ her ¡look ¡like ¡an ¡authority ¡figure ¡and ¡she ¡was ¡clearly ¡viewed ¡as ¡such ¡by ¡the ¡very ¡wary ¡mothers ¡ she ¡encountered. ¡ ¡My ¡informant ¡suggested ¡that ¡uniforms ¡were ¡being ¡introduced ¡now, ¡to ¡help ¡ health ¡visitors ¡feel ¡part ¡of ¡their ¡employing ¡organisation ¡– ¡which ¡implies ¡that ¡the ¡relationship ¡ between ¡employee ¡and ¡employer ¡is ¡more ¡important ¡than ¡the ¡relationship ¡between ¡health ¡ visitor ¡and ¡parent. ¡ ¡Surely ¡this ¡is ¡the ¡wrong ¡way ¡around? ¡ ¡Further ¡discussions ¡with ¡colleagues ¡ reveal ¡that ¡this ¡retrograde ¡step ¡has ¡surfaced ¡in ¡a ¡few ¡organisations ¡recently, ¡not ¡all ¡of ¡which ¡ have ¡been ¡prepared ¡to ¡provide ¡the ¡changing ¡room ¡facilities ¡needed. ¡ ¡ ¡ ¡ Health ¡visitors ¡need ¡to ¡be ¡aware ¡of ¡the ¡‘presentation ¡of ¡self ¡in ¡everyday ¡life,’ ¡which ¡was ¡the ¡title ¡

• f ¡a ¡seminal ¡sociological ¡text ¡by ¡Goffman ¡(1959), ¡whose ¡analysis ¡displayed ¡everyday ¡life, ¡viewing ¡

it ¡as ¡if ¡it ¡were ¡a ¡drama. ¡ ¡He ¡showed ¡how ¡people ¡– ¡the ¡‘actors’ ¡-­‑ ¡all ¡present ¡themselves ¡in ¡ particular ¡ways ¡to ¡achieve ¡the ¡purpose ¡of ¡their ¡interactions, ¡through ¡the ¡way ¡they ¡speak, ¡what ¡ they ¡say ¡and ¡the ¡way ¡they ¡say ¡it ¡– ¡non-­‑verbal ¡communication, ¡including ¡the ¡‘costumes’ ¡they ¡ wear, ¡are ¡a ¡key ¡part ¡of ¡this ¡presentation. ¡ ¡Preparing ¡for ¡a ¡home ¡visit ¡requires ¡care ¡in ¡choice ¡of ¡ costume ¡ ¡– ¡health ¡visitors’ ¡apparel ¡may ¡vary ¡depending ¡on ¡the ¡area, ¡the ¡weather, ¡the ¡kinds ¡of ¡ families ¡and ¡homes ¡being ¡visited. ¡ ¡One ¡health ¡visitor ¡explained ¡to ¡me, ¡years ¡ago, ¡that ¡she ¡always ¡ kept ¡Wellington ¡boots ¡in ¡her ¡car ¡for ¡her ¡visits ¡to ¡traveller ¡camps, ¡since ¡drainage ¡is ¡often ¡missing ¡

¡ 3 ¡ from ¡temporary ¡sites. ¡ ¡The ¡families ¡respected ¡her ¡awareness ¡of ¡the ¡difficulties ¡they ¡faced ¡and ¡ this ¡was ¡a ¡part ¡of ¡gaining ¡acceptance. ¡ ¡Other ¡health ¡visitors ¡have ¡referred ¡to ¡the ¡need ¡to ¡ensure ¡ a ¡neutral ¡appearance ¡– ¡no ¡designer ¡clothes ¡(even ¡if ¡you ¡can ¡afford ¡them!) ¡as ¡that ¡would ¡be ¡ insulting ¡to ¡families ¡struggling ¡to ¡make ¡ends ¡meet, ¡but ¡sufficiently ¡tidy ¡to ¡convey ¡interest ¡and ¡

• respect. ¡ ¡To ¡be ¡practical, ¡clothing ¡needs ¡to ¡be ¡washable ¡and ¡suitable ¡for ¡kneeling ¡or ¡sitting ¡on ¡

the ¡floor ¡to ¡play ¡with ¡toddlers ¡and ¡shoes ¡need ¡to ¡be ¡suitable ¡for ¡standing, ¡walking ¡and ¡driving. ¡ ¡ Stilettos ¡are ¡definitely ¡not ¡recommended. ¡ ¡Presentation ¡of ¡self ¡is ¡a ¡key ¡health ¡visiting ¡attribute, ¡ which ¡is ¡usually ¡taught ¡by ¡example, ¡role ¡modelling ¡and ¡discussion ¡between ¡Practice ¡Teachers ¡ and ¡students. ¡ ¡It ¡will ¡be ¡difficult ¡for ¡students ¡to ¡achieve ¡this ¡experience ¡where ¡uniforms ¡have ¡ been ¡implemented, ¡yet ¡it ¡is ¡an ¡embedded, ¡essential ¡part ¡of ¡practice ¡– ¡like ¡learning ¡to ¡find ¡one’s ¡ way ¡around ¡an ¡estate ¡and ¡care ¡for ¡personal ¡safety. ¡ ¡ ¡ ¡ A ¡key ¡expectation ¡of ¡the ¡Health ¡Visitor ¡Implementation ¡Plan ¡(Department ¡of ¡Health ¡2011, ¡page ¡ 19) ¡is ¡‘to ¡restore ¡professional ¡autonomy ¡and ¡decision ¡making.’ ¡ ¡Choosing ¡which ¡clothes ¡to ¡wear ¡ is ¡the ¡first ¡professional ¡decision ¡of ¡the ¡day ¡for ¡most ¡health ¡visitors, ¡and ¡introducing ¡uniforms ¡is ¡ just ¡one ¡example ¡of ¡how ¡an ¡organisation ¡that ¡fails ¡to ¡understand ¡the ¡culture ¡and ¡nuances ¡of ¡ health ¡visiting ¡can ¡inhibit ¡good ¡practice. ¡ ¡Health ¡visitors, ¡themselves, ¡are ¡often ¡good ¡at ¡ explaining ¡the ¡essentials ¡of ¡their ¡practice ¡and ¡it ¡seems ¡they ¡will ¡have ¡plenty ¡of ¡opportunities ¡to ¡ practice ¡this ¡skill ¡as ¡new ¡organisations, ¡commissioning ¡and ¡management ¡structures ¡come ¡into ¡
