Summary of Solic ita tion

• Procurement Method: Request for Proposals (RFP) – Lowest Price ‐

Technically Acceptable (LPTA).

• Zero (0%) percent LDBE participation requirement, however LDBE

participation is encouraged.

• Performance Period: One (1) year base period from the effective date of the

contract with four (4) one‐year options.

• Contractors are encouraged to ask questions or get clarifications to the

Terms and Conditions during the Q&A period. Any exceptions to the Terms and Conditions must be submitted on the Exceptions to this Solicitation Form (Section X – Attachment 05). Any additional agreements/forms Contractors wish to be incorporated in the Contract must be submitted with the proposal.

Format: PDF, combined into a single file and PDF.

• Do not include any reference to price.
• Submit on typewritten 8 ½ x 11” plain white paper.
• Do not exceed fifteen (15), double‐spaced, single sided pages. Exhibits

and samples of previous work are not included in the 15‐page limit.

• Address the evaluation criteria in the order they are presented.
• Information Security Certification – SSAE or Information Security
