(Be sure to include alternates where required) 5. ENTER ALL CHOICES IN ONCOURSE Have your parent/guardian sign the Course Request 6. Worksheet. Return the Course Request Worksheet to the School 7. Counseling Office by TUESDAY, JANUARY 31st.

Obtain your current teacher’s signature for recommendation. 3. Have parent/guardian sign – “I have read and understand stated expectations of an AP course.” 4. Student signs indicating - “I have read and understand stated expectations of an AP course.” 5. This AP form must be returned with your “Course Request Worksheet.” (JAN 31st)
