Who is Bolton NHS Foundation Trust? • Responsible for providing patient care at - health centres and clinics in the community, at - Royal Bolton Hospital • Also provide: - District Nursing service - Intermediate care (short-term care for patients that are not quite well enough to be at home after a stay in hospital or a period of illness)
