  2. HSE | Open Disclosure Background Patients who are harmed as a result of their health care must be dealt with in an open, honest, empathic and timely manner. They must also receive a sincere and meaningful apology. The new policy ensures that we: • respect the rights of our patients and staff affected by patient safety incidents, • provide appropriate care and support to our patients and staff affected by these incidents • communicate with them in an open, honest compassionate and empathic way, and • treat them with dignity and respect at all times.

  3. HSE | Open Disclosure About Open Disclosure Open disclosure is a core professional requirement which is anchored in professional ethics. We must communicate effectively with our patients in a compassionate, empathic and thoughtful manner at all times and especially when things go wrong. Patients have the right to: • full knowledge about their care and treatment • be fully informed when things go wrong • meet with us to discuss what happened • a sincere apology if an error occurred while caring for them. • be treated with compassion and empathy

HSE | Open Disclosure Policy Requirements The new policy ensures that we: • respect the rights of our patients and staff affected by patient safety incidents, • communicate with them in an open, honest compassionate and empathic way, and • treat them with dignity and respect at all times. HSE | Open Disclosure Our responsibility to our patients Following a patient safety incident it is important to meet with the patient within 24 hours or as soon as possible after the incident to: • acknowledge what has happened, • say you are sorry, • provide the facts available, and • provide reassurance about on-going care and treatment. Open Disclosure may involve meeting with the patient on one or many occasions depending on the level of harm that occurred.

HSE | Open Disclosure Actions • Please read the full policy available at hse.ie/opendisclosure. • be aware of and understand what implications the revised policy has on your work and your patients. • attend open disclosure training recommended by your manager. • take a refresher course in the revised open disclosure policy if you have already received training in open disclosure. Training Open disclosure training is available to all health and social care services. To access training contact the Open Disclosure Lead for your area. A list of Open Disclosure Leads is available at hse.ie/opendisclosure
