EMPLOYMENT STATUS

AT TIME OF DEATH

OFF-PAYROLL When a member is removed from active payroll prior to death, the surviving family is eligible to apply for disability benefits.

INVALID If a valid annuity form is not completed prior to death, up to a 50% JSA can be selected VALID If a valid annuity form is completed prior to death, up to a 100% JSA can be selected ON-PAYROLL If a member is not removed from active payroll prior to death, the surviving family is ineligible to apply for disability benefits.
