and description of the events and report according to the OLF definitions each month.

KPI DEFINITIONS Personal injuries
The Statistics shall include work related deaths and personal injuries. Injuries and or deaths that occur under helicopter transport shall be included. The following parameters shall be included in the Statistics:
- The number of work related personal injuries with decedent
- The number of work related personal injuries that needed more
attention than first aid.
- The number of Exposure hours

The basis for the Statistics:
- 1. The number of personal injuries and the total number of hours of
exposure shall be registered per installation.
- 2. The personal injury frequency is calculated per million hours of work
exposure.
- 3. The incidents real outcome shall be included in this indicator, not its
potential.

KPI DEFINITIONS Personal injuries
These injuries shall NOT be included.
- Injuries that only requires simple first aid
The circumstances of the injury are considered to only require simple first aid. The following are considered as simple first aid.
- Leisure time injuries
8 In General:
- Non prescription drugs in normal non prescription doses.
- Prescription drugs in one dose.
- Tetanus shot / vaccination.
- Heat and cooling treatment (ice bags etc)
- Elastic bandage.
- Temporary immobility in relation to transport of a possible injured person.
- Sting in finger- or toenail.
- Removal of splints or similar objects from other body parts than the eye by simple means .
- Use of protection for fingers.
- Massage.
- Supply of liquids for heatstroke injuries.
All personal injuries that occur in the leisure time (including the personnel's leisure time onboard on the
- ffshore installation). Leisure time is considered as the time outside ordinary shift and overtime. Meals and
breaks are not considered leisure time.
KPI DEFINITIONS
Serious personal injuries (Management regulation §31)
a) head injuries involving concussion, loss of consciousness or other serious consequences b) loss of consciousness due to other causes c) skeletal injuries and tendon injuries, with the exception of rupture or fracture of fingers or toes where the adjoining bones are not out of position (not dislocated fracture) d) injury to internal organs e) full or partial amputation of body parts, with the exception of nails, or the tips of fingers or toes without simultaneous loss of bone substance f) poisoning or chemical exposure with danger of permanent health injury g) burns, frostbite or corrosive injury involving the full dermis (third degree) or partial dermal injuries (second degree) of the face, hands, feet or in the abdomen, as well as all partial dermal injuries affecting more than five per cent of the body h) general hypothermia (second degree or higher) i) permanent damage or delayed consequences of injury leading to defined medical disability, cf. the Directorate of Labour and Welfare’s disability tables j) eye injuries leading to full or partial loss of sight k) ear injuries leading to full or partial loss of hearing l) injury with extensive loss of muscle mass or skin.

KPI DEFINITIONS Hydrocarbon leakage
Hydrocarbon leakage is defined as leakage of Oil, Gas and Condensate from process stream, well stream and flexible
- risers. Chemicals such as Diesel and Methanol are not included. The reporting distinguishes between large and small
leakages.
- 1. Large Hydrocarbon leakage is defined as > 0,1 kg/ sec (combined
total amount of gas, condensate and oil).
The direct cause, amount, installation and date of these leaks shall be reported for the KPI for OLF.
- 2. Small Hydrocarbon leakage is defined as < 0,1 kg / sec of total
amount
These include all other leakages of Oil, Gas and Condensate: 10
KPI DEFINITIONS Dropped objects
A dropped object is defined as a free falling object from one level to another and is classified after the amount of Energy it represents.
The momentum of the falling object is calculated from the following equation:
T x H x 9.8 [m/s2] = [Joule] Where T = mass [kg], H = height [m]
All actual incidents that involve a momentum that exceeds 40 Joule and have a potential for personal injury, shall be reported and is included the statistics. If the dropped objects shape or weight is of such character that it represents a potential for personal injury (in example. Sharp objects, high weight) it shall be reported regardless of its momentum. Note that: An evaluation of the potential for personal injury shall always be performed for all incident involving dropped objects.

KPI DEFINITIONS High risk events
Events that under slightly other circumstances and reasonable probability could have resulted in a major accident.
In example an event that involves serious damage to the installations safety integrity and where there is only one barrier that is intact and can prevent a major accident from
- ccurring.
A major accident is defined as:
- Three deaths or more.
- Material damage for 100 Mill. NOK or more.
(These will be in the red area of the different risk matrix’s that are in use.)
